The settings for the pic are that it's the blood-drinking scene in episode 13, but with the additional AU setting of Kaname having had hidden feelings for Zero.
When Zero drank his blood, he just couldn't go on holding it all inside anymore, and he kissed Zero, much to the hunter's surprise and shock.

Zero is supposed to be shocked, and perhaps also a bit disgusted of himself for having succumbed to the thirst for Kaname's blood. Meanwhile, Kaname is surrenderring to his feelings, and also a little bit desperate, since it's unlikely that his feelings will be returned. (Though in the last variation pic...? XD XD *lol*)

Of course, in my perverted mind, there are big chances that a Zero-goes-crazy-with-bloodlust-and-takes-Kaname scene would follow, hahaha. And Kaname would let him do it because it's his only chance to sleep with him otherwise... ;-;
Except that later, maybe Zero doesn't remembers the kiss etc. and just the end of the sex by when he snapped back to himself, and keeps wondering WHY Kaname let him not only drink his blood but also do that to him. XD XD *lol*
